Heartland - Southwest Sickle Cell Disease Network Telementoring ECHO- FY27

The Heartland-Southwest Sickle Cell Disease Network Tele-mentoring ECHO educational series is planned and implemented inter-professionally by physicians and nurses. This activity is designed for physicians including residents fellows, nurses, and nurse practitioners with specialties in hematology, hematology oncology, emergency medicine, internal medicine, family medicine, pediatrics, neurology, nephrology, and bone marrow transplant. It may also be of interest to physician assistants, psychologists, social workers, and other health care providers who work with patients with sickle cell disease. This 1-hour per session live webinar will provide education through didactic lectures, case presentations, open discussion, and question and answer session. The goal of this activity is enhanced provider knowledge, competency, strategy, skills, performance, and patient outcomes. Each month a tele-mentoring zoom conference will be presented for providers treating patients with sickle cell disease in the Heartland Southwest region of the United States (focusing on Missouri, Nebraska, Iowa, Kansas, Louisiana, Texas, Arkansas, and Oklahoma -- although interested providers from across the nation and world are welcome to join) following the Project ECHO model, a guided-practice model, that reduces health disparities in under-served and remote areas of the state, nation, and world. The ECHO model uses a hub-and-spoke knowledge-sharing approach where expert teams lead virtual clinics, allowing for increased education and sharing of resources to providers in under-served areas so they, in turn, can deliver best practices and evidence-based care to their patients. During the 1-hour ECHO call, an expert in sickle cell disease will share a brief didactic on a topic related to sickle cell care. This is followed by a de-identified case presentation and open discussion and time for questions among attendees. Participants are encouraged to attend the ECHO live, but the didactic presentations from the ECHO are also posted on our website for providers to view at a time most convenient to them.

1. Describe up to date evidence based guidelines for the management of sickle cell disease.
2. Initiate quality improvement activities in their clinical practice in collaboration with other members of the health care team when appropriate to develop evidence based treatment plans for patients with sickle cell disease.
3. Initiate and modify appropriate disease modifying therapies and screening tests for patients with sickle cell disease in accordance with evidence based guidelines.
