Amyloidosis 2027: Diagnostic Dilemmas, Biological Insights, and the Next Generation of Treatment

1. Identify the patient populations in which amyloidosis remains underrecognized, and describe the clinical and structural drivers of diagnostic delay.
2. Interpret mass spectrometry amyloid typing reports, including the significance of bystander proteins, and distinguish amyloidosis from granulomatous and other infiltrative mimics.
3. Apply a systematic approach to the serum free light chain assay and to hematologic and organ response assessment in AL amyloidosis.
4. Describe the biology of the inflammatory and fibrotic myocardial response in ATTR cardiomyopathy and its relationship to heart failure severity.
5. Compare approved diseasemodifying therapies for ATTR cardiomyopathy and outline a framework for monitoring response and escalating treatment.
6. Summarize the ongoing and planned trial landscape in ATTR and AL amyloidosis, including gene editing, nearcomplete silencing, amyloid depletion, and Tcellredirecting therapy.
7. Manage the cardiovascular, renal, gastrointestinal, and neuromuscular consequences of systemic amyloidosis beyond amyloiddirected therapy.
8. Describe practical strategies for initiating and maintaining amyloiddirected therapy, including prior authorization and insurance navigation, and for delivering amyloidosis care within the Veterans Affairs system.
